As a Community Health Center, Cornerstone Care offers a Sliding Fee Discount Schedule to our patients who are income eligible. The Cornerstone Care, Inc. Sliding Fee Discount Program has been developed to prevent a barrier to care. Based on income and household size, our sliding fee schedule may substantially lower your Medical and Dental costs.
In this personal care model, your primary health care provider establishes a team of medical professionals to administer a specialized wellness plan catered to your needs. To achieve this, your healthcare provider will establish a working relationship with your team and will highlight your family situation, medical history and health issues in order to create your specialized plan.
