General Dentistry The most common reason most of us visit a general dentistry office is for our regular check-ups and cleanings. But if those dental check-ups reveal signs of oral disease, we may need to get gum disease treatment. And if oral disease damages our teeth, or if our teeth experience injury as a result of trauma, we may need to get our teeth repaired.
TMD Treatment Jaw pain, a sore neck, constant headaches, or an intense earache are sure signs that something isn’t right. If you’re living with any of these painful symptoms, you may have a TMJ disorder, often called TMD. Thankfully, there are TMD treatment options available to help.

TMD is a disorder of the temporomandibular joint – the jaw joint. The problem is most often caused by your bite. Your “bite” is a delicately engineered machine. The position of your jaw joints, and the way your jaw muscles work and your teeth fit together are critical for a healthy bite.

If any one of them is out of whack, eventually the machine will break down. TMD can cause damage to your teeth (wear, fractures, chips) , gum tissues (recession and bone loss) , jaw joints (arthritis, noises, locking) , and muscles (pain) .
Mercury Free Dentistry One argument that is sometimes used to defend the use of mercury fillings it’s locked in the fillings. This is not true. There are several mechanisms by which it can exit the fillings.

First, it is a liquid metal, and it can evaporate, creating vapor in the mouth that is then inhaled. As a liquid, it is also able to seep through the tooth material. This not only discolors your tooth, it allows mercury to reach the bloodstream via the tooth pulp. Finally, chemical reactions in the mouth create an electric current similar to a battery. As part of the corrosion that occurs, it can be liberated from your fillings.

Experiments with sheep have shown that it only takes a few months for mercury to spread to the entire body, with the highest concentrations found in the kidney and liver. Pregnant sheep also showed that it passed into the fetus, concentrating in the liver and pituitary gland, which has a central role in development. Tests of both sheep and human breast milk showed that mercury could be found there as well, and that the concentration levels related to the presence of metal fillings.
OMD Treatment Now that you recognize some of the symptoms that could indicate you have an OMD, you might wonder, how did I develop this condition?

OMDs occur when the normal developmental pattern of your face and jaw is interrupted or altered, usually by certain physical structures or habitual actions. Facial development is governed by the relationship between your muscles (including the lips, cheeks, and tongue) , teeth, and jaw. Dynamic, repeated pressure from the muscles stimulates the hard structures to grow, but also limits their growth. It’s the balance between these forces that governs development.

The normal balance can be altered by some bad habits, including:

Thumb or finger sucking
Mouth breathing
Biting nails
Chewing nonfood objects
Tongue thrusting
Inadequate chewing or avoiding hard foods
Poor swallowing habits
But it can also be changed by unusual physical structures such as a very restrictive tongue or lip tie.

Often, these habits and structures create a reinforcing pattern that can intensify with time. For example, a tongue tie might keep the tongue from stimulating development of the hard palate of the mouth. The hard palate is the roof of the mouth, but also the floor of the sinuses. If it doesn’t develop properly, the nasal airway may be narrow and inadequate. To supplement air from the nostrils, a person begins breathing through the mouth. In order to breathe through the mouth, the tongue must be on the bottom of the mouth, which means it provides even less stimulation to grow the upper palate, which results in improper chewing and swallowing.
Dental Implants Dental implants are a welcome alternative to poor-fitting dentures or bridgework. Approximately 21% of all Americans are missing some or all of their teeth. Wearing a complete denture can cause gum loss, which often results in pain or discomfort. When loose dentures rub against your gums, the pain caused by the abrasion takes the joy out of pretty well everything that involves your teeth. Eating is not just a way to stay alive, in almost every human culture, eating is a social, happy event.

Fortunately, modern technology has advanced dental implants to the point where they can be used to replace as many missing or broken teeth as you have, even if you have lost all your teeth. These days, we’re living longer. With dental implants you can live your whole life with the greatest possible pleasure.

Once installed, your dental implants will give your new teeth solid support. Your bone heals around the root device, essentially becoming part of the structure of your jaw and face. When you use dental implants to replace missing or broken teeth, your face gets the structural support it needs to keep its natural shape and beauty.
Dentures Dentures with a Comprehensive Fit
The reason why many dentures fit or function poorly is that dentists don’t fit them properly. Economy dentures come in just a few sizes. A dentist selects the one closest to the size of your mouth, then takes an impression and makes sure they fit over your gums. Essentially, these fit only your gums. Dental Bridges Dental bridges will do all three things for you. At Top Down Dental in San Jose/Los Gatos, Dr. Nancy Nehawandian is trained and experienced in both cosmetic and general dentistry.
With one eye on your beauty and one eye on your health, Dr. Nancy Nehawandian will recontour your abutment teeth to prepare them for crowns that will support the dental bridge. Next she’ll take an impression of your teeth and send it to a dental lab who will use it as a model to make your bridge. While your dental bridge is being fabricated, you’ll be fitted with a temporary bridge to protect your teeth and gums.
When your permanent dental bridge is completed, you’ll head back to Top Down Dental in San Jose/Los Gatos to be fitted. An excellent fit is critical for your smile, oral health and comfort, and Dr. Nehawandian may schedule subsequent visits to make sure you get the best fit possible.
Tooth-Colored Fillings Tooth fillings are used when dental decay has created a cavity in your tooth. Left untreated, the cavity can shelter bacteria and trap food, causing the tooth to decay even more. Eventually, the cavity will pierce the inner chamber of the tooth, where the living portion of the tooth, called the pulp, is located. When bacteria infect the tooth, it may need to be extracted. Fillings are an early preventive treatment to preserve teeth. And these days they are attractive as well as functional.

The two different white filling options can help you get the optimal treatment for your teeth. Dental fillings can be made of a composite that is made of a plastic filled with hard ceramic particles that give it strength. These fillings are inexpensive and easily placed in a single visit. They polish nicely to emulate the luster of your natural teeth. But they may not last as long as we want. They can lose their polish, may become discolored, and can wear out.

Ceramic fillings are more of an investment of time and money, but they can be worth it. They are the most attractive fillings available, and they are the longest-lasting. The only drawback is that they require two visits to place, one where your tooth is prepared and one where the filling is placed.
Porcelain Veneers Generally, dental veneers cost less than other cosmetic treatments, because the process is less invasive and less time consuming. Porcelain veneers can usually be installed in just two visits to Top Down Dental in San Jose/Los Gatos.

On your first visit, Dr. Nancy Nehawandian will prepare your teeth by buffing them lightly. Next she’ll shape the surface of your teeth so that the veneer will adhere well. After she takes a mold of your teeth, you’ll help choose the color.

You’ll be fitted with temporary veneers until your permanent ones are fitted. The temporaries allow you and Dr. Nancy Nehawandian to assess the look and fit of your veneers, ensuring your permanent veneers are comfortable and attractive.

Once your porcelain veneers are fabricated, you’ll come in for your second visit – the installation. The staff at Top Down Dental will clean your teeth with a special solutions that will help the adhesive form a durable bond. The veneer is cemented in place using an adhesive hardening light.

That’s it. Requiring little or no anesthetic, the whole process is quick, painless, and effective.
Full Mouth Reconstruction A full mouth reconstruction is recommended when you have a collapsed bite. This means that your teeth are not properly meeting together and not properly supporting the lower third of your face. Your teeth may be worn, they may be lost, or they may just not be coming together properly. As a result, your teeth may be painful, your jaw may hurt when you bite and chew, you may have chronic headaches, and your face can look sunken and imbalanced.

With older techniques, where the mouth is restored one tooth at a time, it takes multiple visits, the old collapsed bite is duplicated, and the appearance of the face is not improved. Neuromuscular Dentistry allows Dr. Nehawandian to establish the most comfortable bite position using computerized neuromuscular technology. This includes tools to measure the movement and function of the jaw, including determining the stress in your muscles. We also use T-Scan, a digital tool that shows us exactly how much force your teeth are subjected to as you bite down. This allows us to ensure that every system from your muscles to your teeth are all working together in balance.

Dr. Nehawandian has received extensive training in full mouth reconstruction at LVI. It can dramatically improve your bite as well as the appearance of your smile and your face.
Ozone Therapy Ozone can be thought of as an activated form of oxygen. Most oxygen is in stable molecules with two oxygen atoms, but ozone is a molecule with three oxygen atoms, which makes it unstable. As an unstable molecule, ozone wants to react with other molecules around it, especially organic molecules. This means that it is very good at attacking oral bacteria and killing it. Which makes it a great sterilization treatment in many environments.

Ozone is also a great sterilizer because as a gas it can penetrate into areas where we can’t clearly see, let alone shine a laser. And ozone has other valuable properties for certain applications.

Ozone Can Reverse Minor Tooth Decay
Tooth decay is caused by oral bacteria which excrete acid on teeth, removing vital minerals from the enamel or dentin. The minerals that make tooth enamel strong against pressure make it weak against acid. But ozone is a powerful treatment for reversing the process of decay.

Of course, when we blow ozone on an area impacted by decay, it kills the oral bacteria there, stopping current decay.

But as ozone decays, it doesn’t just turn into oxygen (which is deadly to many of the most harmful bacteria, but harmless to you) , it bonds with hydrogen atoms in the environment. Loose hydrogen is what makes a solution acidic, so as ozone is bonding with these hydrogens, it is neutralizing the acid and creating a basic environment.

In basic solution, the process of tooth decay reverses. The tooth takes minerals from saliva or from a mineral-rich formula we apply to the tooth, repairing the early stages of tooth decay. This stops or postpones the need for a dental filling.

Should have sought a second opinion. Very aggressive in diagnosis and treatment - and even more aggressive with a drill. At least I didn't pursue the other 2 crowns that were recommended - those I did get a 2nd (and 3rd) opinion on and they were completely unnecessary. The one I did get placed I've had to spend several thousands of dollars on, including the original placement and getting it fixed with another dentist.

The thing that gets me is I didn't need a crown at all in the first place. Great hygienists and office staff, but I would not trust beyond a basic cleaning. Run away, fast.

I have no idea how anyone could have had a horrible experience with Dr. Nehawandian based on my experience. Background - I've been seeing a functional medicine doctor for chronic gut, fatigue and hormonal issues so looking for a holistic/biological dentist was at the top of my list since the mouth is a crucial part to the gut. When I first visited Top Down Dental they were incredibly thorough with their examination and I decided to do a bacterial test to get more tangible results.

She recommended that I do a deep cleaning/laser treatment because based on her findings, I have the beginnings of periodontal disease which is hereditary in my family. I started second guessing myself because the treatment plan wasn't cheap and truthfully, I felt nervous about how much the deep clean could hurt because of past dental experiences.

I went and got a second opinion at another dentist that shared they thought I needed just a standard cleaning. I felt relief yet a nagging feeling because I've been through so much with my health and something was telling me to share this with Nancy and her team. I also hadn't gotten my bacterial results back yet. I was more than just pleasantly surprised with the level of care and integrity I experienced connecting with them.

I shared my concerns about the price and the second opinion I received and had an in-depth conversation with Nancy. She shared (both on the phone and the results in email) that the bacterial level I had was way above safe levels and probably in my bloodstream. Additionally, the reason she recommended the laser treatment was because it would kill the bacteria, close the doorway to bacteria getting into my gums and bone and when the machine she uses measured the pockets in my gums they were a level 5 (the other dentist said 4 but that was eyeballing it.) I was so grateful to have both a qualitative and quantitative approach to my dental health.

Not only that, but both dental hygienists that did my treatments were PHENOMENAL (thank you Julie and Alexis!) and while there were moments of pain, I can tell that my body is learning that it's safe to go to the dentist and I am even looking forward to going back. There is a noticeable difference to my breath, my body and my resolve to take better care of my teeth and gums.

They also made it easy to pay for the treatment by offering health credit loans with ample time to pay back. As someone who currently is working minimally because of my health issues, this was incredibly helpful. I know that investing in myself in this way was worth it. Thank you Dr. Nehawandian and team!

I can't say enough good things about Dr. Nancy and the entire staff at Top Down Dental. They go above and beyond to make sure you get the best quality care, and it's always a joyful experience visiting their office. If you live near Los Gatos go see Dr. Nancy!

I had a horrible experience with Nancy. I had a crown put in and I'm now in constant pain. I went to another dentist who found two sections of my neighboring teeth taken off. We compared x-rays prior to the crown placement and after to confirm that in fact my neighboring tooth was damaged. Nancy was so rough during the procedure. She never mentioned that in the process she damaged my other tooth.

I'm now in constant pain. My new dentist was shocked at the work that Nancy did and even pointed out a section where my crown is jagged on the x-rays and doesn't lay flush with my tooth. She said this will likely cause cavities in the future. I am so disappointed that I went to Nancy and will be reporting her to the appropriate licensing boards as I really think Nancy should not be practicing as a dentist.

The office is nice inside with the dim lighting and massage chairs but if you want a knowledgeable dentist that is going to do good work this is not the place. I had several fillings done that ended up having to be replaced by another dentist. The second filling ended up needing a root canal. I've had fillings before but find Nancy to be extremely rough.

She just doesn't have a gentle approach. I'd rather do without the massage/spa environment and have a competent dentist do work on my teeth. Please do yourself a favor and find one of the many excellent dentist in the Bay Area not this place. I'm extremely happy with my new dentist and above all they seem knowledgeable and trustworthy. Avoid Top Down Dental at all costs!

Please please don't go here. Nancy caused me a lot of pain and suffering when the anesthesia shot she gave me I believe hit a nerve. I've been in constant pain since then. I've had numerous trips to the neurologist since then with little relief. She seemed distracted and clumsy when she was working on my teeth. I've paid the ultimate price now because I'm in constant pain. Eating and chewing are unbearable. I wish I would have never went to Nancy. The dental work she has done on my teeth has ruined my life.

Is Nancy a real dentist? She caused me so much extreme pain and suffering. She placed a crown on my tooth that was not placed properly and it needed to be replaced after only 3 months. I was in constant pain after Nancy placed the crown. She seemed distracted when she placed the crown and was too busy watching the show that was playing on the screen then looking at my tooth. I am shocked that she is a licensed dentist. I'd encourage anyone looking for a new dentist to go elsewhere.

Unfortunately I cannot recommend Top Down Dental. The dental hygienist does a fairly good job cleaning but the problems lie in the dentist Nancy. She does not appear current or knowledgeable in her trade. On multiple occasions she recommended outdated procedures and when asked she wasn't aware of the current standard of care. I brought this to the attention of my new dentist who was amazed at some of her recommendations and techniques. Although the office feels fairly new the dentist is not up to date at all in her profession. I would go somewhere else.

I had a total overhaul of my teeth in 2016. All teeth got crowns. It cost me a fortune. Four teeth got root canals. It was a big ordeal that lasted almost a year. I have been taking care of my new crowns as I was told. Stopped smoking as well. One tooth went bad, so I got an implant, which I lost, although the dentist said it was clear to implant.

It just fell out at night. I suspect it was because there was nickel involved, and I am highly allergic to it. Well, I was told they could fix it, but it would have cost me another thousand or more. No warranty. I now lost TWO more teeth, they just broke off. I am now in Germany and cannot sue. I feel betrayed, especially because I was acquainted with the dentist. I have to second the review that mentioned BUYER BEWARE! Nancy and her team are terrible. I had a small crack in my tooth and was pushed into getting a crown by Nancy, so she could make more money off of me. She did a terrible job and caused me to need a root canal, not to mention the horrible constant pain. I have three friends that have also had botched procedures by Nancy.

In my case, she damaged a nearby tooth because she was sloppy during the procedure and not paying attention. I have the before and after x-rays to prove it. I truly believe a place with dim lights and message chairs is not where you want to go for quality work. Top Down Dental is nothing but a gloried Message Envy with incompetent staff. If you want a massage and movie go to Top Down Dental but if you want a real dentist who will perform HONEST and quality work choose a different place. She is very overpriced and has the one goal of making money. There are plenty of honest and good dentists out there unfortunately Nancy is not one of them.

I feel like the most blessed person in the world to have found Dr. Nancy and her fabulous staff at Top Down Dental. I've had a lot of traumatic experiences throughout my life with dentists and dental procedures, but the sincere concern and top notch care provided by Dr. Nancy and her team are bar none. I literally feel like I am going to a spa or being treated like royalty when I go in there.

The massaging chairs, ambient music via headphones, heated neck pillows and more are provided for you while you are being worked on. This is so relaxing, healing and really helps ease the pain of having to get any array of dental procedures done. Besides the amazing amenities, Dr. Nancy Nehawandian is one of the most intelligent, in tune, nurturing doctors I've ever met- in any medical profession.

Beyond her many traditional certifications, she offers a holistic approach to provide her patients with the best possible care that is up to date with the times, techniques and technology. She even offers ozone therapy in her treatments! I simply cannot say enough wonderful things about Dr. Nancy and the incredible people she has on her team.

I recently switch to Top Down Dental after my former natural/holistic dentist quit taking my insurance (I was happy with him). I researched extensively to find a Dr. that would be more natural/holistic and not just treat "symptoms" as well as using non-toxic materials. I found Dr. Nancy and have been very impressed with her knowledge, experience but mostly her patient way of explaining treatments and recommendations. The visits are very "gentle" and the experience is "spa like".

Buyer Beware! I went to Dr. Nancy Newandian for several years because I needed a sedation dentist and recently left when my hygenist left. I never much cared for the dentist herself -- she was not warm and friendly. Her exams last 2 minutes -- my last exam consisted of her looking at the xrays and talking about sleep apnea. My first visit she told me I urgently needed full mouth planeing and scaling to protect my teeth and I did that under sedation.

She charged me $7000 which, in my stupidity I did not shop. I found out today that my current sedation dentist, more established and with a better reputation, charges $2000 for that procedure -- 5 years later! I feel taken and cheated.

I have been going to Dr. Nancy for the past 5 years. She and her staff go out of their way to make you feel welcome and comfortable while you're there. From the relaxing massage chairs, relaxing lotions, and steamed hand towel infused with lavender oil, this dentist office feels like a spa. I highly recommend going to Dr. Nancy as she is experienced, knowledgeable and really kind. She and her team are the best!

All the bells and whistles of a modern dentist office. Aggressive push for an expensive treatment plan was off putting. Of the four plus crowns/root canals, all had issues fixed by another dentist. Three crowns came off (two within six months). One of these revealed a botched root canal done at Top Down that resulted in tooth removal. That particular root canal was done twice at Top Down as the first attempt did not clear things out. Apparently, neither did the second. Nice people but workmanship/materials issues make this place an incredibly risky dental option. Go elsewhere.

WHO LOVES THEIR DENTIST?! I do. Never healthier, nor so pain free. I am grateful to have discovered them when I had to find a new dentist years ago and have been blessed with healthy gums ever since! They performed deep cleaning and recommended a treatment that has since literally eliminated sensitivity (!), improved the health of my gums and teeth, and likely my heart as a result.

The office staff are as caring as Dr Nancy, herself, and I am treated like family Since relocating, I will be driving to the bay area twice a year for regular visits, as in 55 years I've never found any health practice like this one - one of a kind

I have been going to Top Down Dental since I was a child, and after my wonderful experiences I will continue to do so as I enter the threshold of adulthood. The experience is relaxing, and catered to the patient. 1. Parking is never an issue in the spacious lot in front of the office. 2. Entering the office, you walk into the clean and modern waiting room and settle into a large armchair.

The amazing Vicki is at the front desk, greeting you with a smile. There is coffee and tea, juices in the fridge, and usually some type of cookie for your snacking enjoyment. Usually the wait isn't long, so you don't get to spend much time in the waiting room. 3. For patients who experience anxiety at the dentist, there is a serene room with technology like NuCalm and a massage chair. 4.

Typically a hygienist will come to retrieve you, and take you to your room. A fun bonus: if you want, you can choose to watch a movie during your appointment! The rooms are comfortable and clean. You take a seat into the comfortable chair, facing the nature of Los Gatos. 5. Depending on the type of appointment you scheduled, either you will be greeted a gentle but methodical hygienist (the wonderful Briar or Kim) or by the experienced and thorough Dr. Nancy. Dr. Nancy is extensively knowledgeable in her practice.

One of the best dentists in the area, she is detailed and methodical in her practice, and stays on top of new dental technologies. More than that, she is caring and kind with her patients. The services she offers have helped my whole family; my brother's breathing problems have been alleviated with a night time mouth guard, and my mother's chipped crown from a ski accident was elegantly repaired by Dr. Nancy.

Her smile looks better than ever. Another bonus: the experience is relaxing and fun for children as well! The movies are an exciting distraction and as a child I was offered small toys, 'treasures, ' from a treasure chest after appointments. Overall, Top Down Dental is simply a fantastic gem of a dentistry in Los Gatos. I plan to continue visiting their practice as long as I live in the Silicon Valley.

I started seeing Dr. Nancy after a recommendation from another fantastic dentist I know (who is now retired) from another state. When I started seeing her, I had multiple problems in my tooth due to an implant badly done. Dr Nancy was able to make me recover from that situation quickly and make recommendations which would make my general oral health much better in the long run. It was a complete 180 turn for me. I completely recommend this place. To top it all, the staff is amazing!

I have been going to Top Down Dental for about twenty years now. They are knowledgable and courteous. Sometimes it feels like they are recommending expensive services. $5000+ for a highly customized mouth guard, etc. They seem a bit over-priced to me in that regard but that just might be my perception. Overall, for quality of service, I would absolutely recommend them.

Dr. Nehawandian has been my dentist for years and I can't imagine going to any other dental office than Top Down Dental. I typically get nervous before going to the dentist but everyone at this office makes me feel at home. At the office, they have different systems and tools that make you feel relaxed before your treatment. During my procedure, I typically enjoy a movie and use the massage feature on the chair.

The hygienist is very friendly and knowledgeable. She spends lots of time educating me about the importance of healthy teeth and gums. Dr. Nehawandian always explains any necessary procedures. She takes the time to explain my options to me and will answer any questions that I have. I love how everyone who works there takes the time to get to know me and my health personally and is extremely friendly every time I visit.

Be careful. I came here in lots of pain due to a neck injury. Someone recommended I get checked for TMJ. After about an hour of tests and images that did not seem to go smoothly, the doctor was convinced I had TMJ and we started the path to get a mouthpiece made. The mouthpiece came in, and it didn't feel right. I had to go back in multiple times, where the piece was sawed down, but still never seemed to fit. Basically, this threw my bite WAY off, caused more pain, nightly clenching, and has resulted in lots of money, time. and energy put into having to get my bite readjusted by an osteopath.

Please be careful before agreeing to any treatment, especially if you are in a vulnerable state like I was. I am still paying off an unfair bill, and was left with more pain and frustration than when I came in. I do think they mean well, but should stick to regular dentistry. Beware.

Love Dr. Nancy and the entire team. Who would have thought that you can look forward to going to the dentist?! I started to come here about 2 years ago because I had terrible TMJ, no dentist that wanted to treat it or was willing to even acknowledge what I have and no advice I felt I could trust. After much online research I found Dr. Nancy. First she took pictures of my posture, then of my mouth and ran numerous tests.

Then I got braces and the TMJ is much much better. Treatment is not complete yet, but I can feel the improvement and feel like it has been a success already. I can only recommend her as a dentist. Not only does she look at your teeth, but more your body as a whole and how other parts may actually impact your bite. She has the latest technology in her office so that she can really treat whatever you may have.

I feel like I can trust her and she is honest with me, which is definitely key with dentists. Also, she really explains to you what is going on. she shows you pictures and xrays on a big screen to show you the problem and makes sure you understand what you are paying for. :) The staff is also amazing. Vicky and Fey are so nice. They always greet you with a smile, remember you, know stories and make it just a nice experience to be there.

The office is beautiful and warm. And if you are lucky, you even get to sit in the massage chair and watch a movie for a bit while getting treatment. :) If you can't tell, I absolutely love it and can only recommend it. I don't know if it is more or less expensive than other dentists. but for me going there is worth every penny. :)

5 Stars! The Staff and Dr. Nancy Nehawandian of Top Down Dental are the nicest and most professional dental team in the Bay. As soon as you walk into their waiting room, you are greeted, and can hang out in their comfortable waiting room. During uncomfortable procedures, Dr Nancy allows you to watch a movie or use the NuCalm system, where you take minerals and listen to relaxing music.

Between teeth cleaning (6 months), Dr. Nancy finds ways to improve & innovate her business. One of my favorite new features is text reminders of when my next appointment will be, so helpful! I highly recommend Top Down Dental to anyone in search of the right dentist. Dr Nancy is a great dentist and all her employees are warm and friendly!
