We're anxious to meet you, but first, a little paperwork! We'll need you to come early to complete all insurance, health, and patient information forms that will allow us to begin your dental treatment. You can get a head start by downloading and printing the forms below. In most cases, we can address concerns and create an individualized treatment plan on the same day.
At Broadway Dental Excellence, we love seeing you! The American Dental Association recommends that you see your dentist at least twice a year for preventive care. By checking your mouth every six months, we can often reduce your potential for larger dental problems. Small cavities can grow to destroy an entire tooth.
Everyone over the age of four should visit the dentist for dental cleanings twice per year. At Broadway Dental Excellence, we have four specially-trained hygienists on staff to ensure convenient scheduling of regular appointments for all of our patients. Your hygienist will begin your cleaning by exploring the surface of your teeth to determine if you have cavities and examine the quality of existing fillings. The hygienist will then inspect your gums for periodontal disease, the leading cause of tooth loss for American adults.

Jan 2017 mine husband needed emergency dental work while an inpatient at EMMC. Dr Tozer came in to do what should have been a quick job, turned into 13 hours long. He ended up coming back to the hospital to get bleeding under control. The teeth extraction was a difficult one. But Patient, Dr and family all pulled through. His bedside manner and personality are TOPS!!!!! Thank you for taking duck good care of my hubby!!!

Broadway dental excellence is the only place I will go. They are so gentle and they all have such great personalities, so caring and fun. I recommend this dentist to everyone! Broadway Dental is the best of the best. I now live in Boston and still commute 200mi up i95 to not only see Dr. Tozer and Linda, but also Nancy and Kathryn at the front desk! Today I had my first cavity (I am still in denial), and Dr. Tozer went out of his way to slide me in for the same day to get it filled. Who can honestly say that they enjoy going to the dentist? I absolutely can, and really appreciate the bright smiles of the team at Broadway Dental! Thank you all!!
