This information will allow us to develop the right treatment plan for your specific smile goals and needs. You can fill out this information in a "New Patient Information Form" before your appointment.

Dr. Young is simply amazing. My two kids absolutely love going to Colts Neck Center for Orthodontics & Invisalign. The staff is friendly and knowledgeable. I always feel welcome and happy with the service here. Dr. Young takes the time to go over the treatment plan and is always available to answer any questions or take of an emergency. Thank you for helping my kids
